Please note that Eye Care Center patients are seen by appointment Monday–Friday between the hours of 8am and 5pm. Prompt care for urgent issues is also available.

Referral Information

The referral is to be filled out by the referring provider and faxed to the fax number listed above.
We will then contact the patient to schedule an appointment.

 

Location Information

 

The Eye Care Center is located only at the Pomona PCC,on the 1st floor, Suite 2.

 

*Eye Care services are not available
 in our Rancho Cucamonga location.
