Make an Appointment at the Dental Center

The first appointment for all patients consists of an initial evaluation to ensure the Dental Center can meet your dental treatment needs. This evaluation will include a review of your health history, visual screening of your teeth and x-rays. When you call to make an appointment, you will be told about the information you need to bring with you and the fees you will need to pay for this initial evaluation.

Dental Center patients are seen by appointment
Monday–Friday between the hours of 9 am and 4:30 pm.
Prompt care is available for dental emergencies.

Referral Information

The referral is to be filled out by the referring provider and faxed to the fax number listed above.
We will then contact the patient to schedule an appointment.

 

Location Information

 

The Dental Center is located in the Pomona PCC only, on the
3rd floor, Suite 8.

*Dental services are not available in our Rancho Cucamonga location.
